Job Description:

Gateway Community Health Center is seeking a skilled Patient Navigator/Driver to join our team. As a Patient Navigator/Driver, you will play a critical role in providing field-based navigation services to address the social determinants of health within patients enrolled in our Community Mental Health Grant Program.

Key Responsibilities:

  1. Work closely with the interdisciplinary team to provide navigation services to patients.
  2. Assess and recognize the necessary treatment and management for patients, as well as any primary care services needed.
  3. Develop a case file for each patient, identify the primary care and mental services needed, and facilitate referrals.
  4. Ensure a smooth transition for each patient from one healthcare provider or setting to another.
  5. Maintain confidentiality per HIPAA guidelines in regards to patient information.
  6. Perform other duties as assigned.

Requirements:

  1. Bachelor's degree in Social Work, Sociology, Psychology, Health Education, or a related field.
  2. College education may be substituted for three years' experience in a healthcare setting.
  3. Ability to work long hours during the week and weekends as necessary.
  4. Bilingual in English and Spanish is preferred.
  5. Valid Texas Driver's License and minimum liability insurance.
