Responsibilities include implementing physician-prescribed plans of care, conducting patient assessments, administering medication and treatments, and maintaining accurate documentation. We also prioritize health promotion, teaching, and training family members on proper care and management.

This role requires the ability to work independently, communicate effectively with patients, families, and healthcare teams, and maintain flexibility to meet changing patient needs. A strong commitment to quality, safety, and patient-centered care is essential.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as opportunities for professional growth, development, and advancement within the organization.
