Job Summary

We are seeking a highly skilled RN Care Manager to join our team. As a RN Care Manager, you will play a critical role in supporting patient care, coordinating services, and optimizing care delivery. Your expertise will be instrumental in crafting personalized care plans, conducting regular assessments, and proactively engaging with patients and healthcare teams.

Key Responsibilities
  • Develop and implement comprehensive care plans addressing clinical and non-clinical needs
  • Conduct regular assessments to detect medical or social issues and changes in the home environment
  • Initiate proactive outreach to patients and clinics to evaluate their status and offer assistance
  • Manage a diverse patient panel, evaluating individual needs and adhering to NCQA standards
  • Communicate updates to the care team and family members, delivering tailored education and resources
  • Act as a liaison with dialysis facilities, conveying care plan updates and maintaining accurate program software records
  • Generate and evaluate reports using Strive data platforms to identify trends and areas requiring focus
Requirements
  • Associate's degree from an accredited school of nursing
  • Current Registered Nurse (RN) license in the state of practice
  • 3+ years combined of related education, experience, or certification
  • Current BLS or CPR Certification required
  • Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region
Preferred Qualifications
  • 3+ years experience in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing
  • Certified Case Manager (CCM) certification preferred
  • Proficiency in computer skills, including Microsoft 365 Suite, due to working in a hybrid-remote environment
