CNA Hospice Care Specialist

As a CNA Hospice Care Specialist at Corewell Health, you will have the opportunity to provide compassionate end-of-life care to individuals with a life expectancy of six months or less. Our team specializes in delivering high-quality care to patients in their homes, nursing facilities, and hospitals.
You will work closely with an interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient's end-of-life care needs. Your goal will be to create an outcome-based plan of care that ensures a symptom-free, peaceful, and sacred death.
Responsibilities- Provide direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit.
- Responsible for direct day-to-day care of assigned residents/patients, including dressing, bathing, transporting, and feeding.
- Document care provided as directed in a manner consistent with guidelines.
- High School Diploma or equivalent
- Completion of State of Michigan approved CNA Training Course
- PROMETRIC Certified Nursing Assistant required within 90 days
- Driver's License within the State of Michigan
