Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at LHC Group. As a Home Health Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

About the Role:
  • Evaluate patient care needs and develop personalized treatment plans to improve functional status, mobility, and overall well-being.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide expert physical therapy services to patients with various medical conditions, including pain management, mobility issues, and other functional limitations.
  • Conduct thorough assessments and evaluations to identify patient needs and develop targeted interventions.
  • Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
Requirements:
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable transportation.
