We are seeking a dedicated and skilled Physical Therapist to provide high-quality patient care in our dynamic outpatient setting.
Job SummarySilver Creek Physical Therapy is a premier provider of outpatient orthopedics serving San Jose and surrounding communities for over 20 years. As a Physical Therapist, you will conduct comprehensive assessments, develop tailored treatment plans, and help patients achieve their rehabilitation goals.
Key Responsibilities- Conduct thorough patient evaluations to assess physical conditions, mobility, and functional abilities.
- Develop individualized treatment plans based on evaluations and patient goals, incorporating evidence-based practices.
- Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
- Maintain accurate and timely documentation of patient progress, treatment sessions, and outcomes in compliance with regulations and clinic policies.
- Educate patients and their families about rehabilitation processes, home exercise programs, and preventative care strategies.
- Regularly assess and adjust treatment plans based on patient progress and feedback.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current state licensure or eligibility for licensure as a Physical Therapist.
- Previous outpatient experience preferred but not required.
- Ability to work collaboratively in a team-oriented environment.
