About the Role

We are seeking a highly motivated and experienced Case Manager, RN to join our team in Delaware. As a key member of our healthcare team, you will be responsible for providing community-based care coordination and management services to our clients.

Job Summary

This is a full-time hybrid position that requires traveling to members' homes, nursing facilities, and other community-based settings to conduct face-to-face needs assessments and provide telephonic support. You will assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex health care needs. Your goal will be to facilitate authorization, coordination, continuity, and appropriateness of care and services in community or HCBS settings.

Key Responsibilities
  • Travel to members' homes, nursing facilities, and other community-based settings to conduct face-to-face needs assessments and provide telephonic support.
  • Assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex health care needs.
  • Facilitate authorization, coordination, continuity, and appropriateness of care and services in community or HCBS settings.
  • Collaborate with the member's healthcare and service delivery team to coordinate care needs and community resources.
  • Educate members and caregivers regarding healthcare needs, available benefits, resources, and services.
Requirements

To be successful in this role, you must have:

  • A current Registered Nurse license in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning, including experience discharging members from a facility setting.
  • Experience completing assessments, developing service plans, and care plans.
  • Ability to communicate clearly and effectively with members, caregivers, and healthcare providers.
  • Proficiency in PC-based word processing and database documentation.
  • Reliable transportation to travel within assigned territory.
Salary Range

The estimated salary for this role is between $65,000 and $85,000 per year, depending on experience and qualifications.
