Job Overview

We are seeking a highly skilled and dedicated Qualified Professional to join our Community Support Team at Carolina Therapeutic Services First. As a key member of our team, you will play a vital role in providing comprehensive support services to individuals with mental illness and substance use disorders.

Key Responsibilities
  • Provide psychoeducation and assist with crisis interventions
  • Assist with behavioral and substance use disorder treatment interventions
  • Develop and implement relapse prevention and disease management strategies
  • Participate in the development, implementation, and revision of Person-Centered Plans (PCPs)
  • Communicate client progress and effectiveness of strategies to the Team Lead
  • Coordinate initial and ongoing assessment activities
  • Develop and implement PCPs and ensure their implementation
  • Consult with medical and non-medical providers, engage community and natural supports, and include their input in the person-centered planning process
  • Ensure linkage to clinically appropriate and effective services, including arranging for psychological and psychiatric evaluations
  • Monitor and document client progress and effectiveness of strategies
  • Complete functional needs assessments to determine scope and anticipated outcomes of services
  • Linkage and referral to formal and informal supports
  • Monitoring and follow-up
Requirements

We are seeking a candidate with a Bachelor's degree and 4 years of experience working in a human service-related field, or 30 semester hours in a human services field and at least 1 year of experience in delivering services to the population groups we serve.
