About the Role:

Key Responsibilities:
As a Medical Social Worker Specialist, your primary focus will be on providing high-quality social work services to patients in their homes. This will involve conducting comprehensive evaluations of the psychosocial status of patients and their families, developing social work plans of care, and providing counseling and support as needed.
Other key responsibilities include:
• Completing discharge planning and referrals for patients and their families;
• Documenting all services provided according to state and federal regulations;
• Managing caseloads and prioritizing tasks effectively;
• Participating in interdisciplinary team meetings and contributing to the development of patient care plans;
• Staying up-to-date with professional skills and knowledge through ongoing education and training.
Requirements:
To be successful in this role, you will need to possess a Master's degree in Social Work from an accredited college or university, as well as a valid license from the State of CT as a Licensed Clinical Social Worker. You will also need to have at least two years of clinical experience, preferably in home care. In addition, you will need to have adequate transportation and proof of satisfactory automobile insurance.
