Care Transition Specialist II

Job Summary:
The Care Transition Specialist II plays a vital role in ensuring seamless transitions of care for patients, working collaboratively with the care team to complete administrative responsibilities related to care progression and transitions along the continuum of care.
Key Responsibilities:
- Provides direct administrative support to the care team, patients, and patients' caregivers related to continuum of care.
- Collects, confirms, and verifies key patient information, including demographics, healthcare proxy, benefit verification, and patient preferences for pharmacy, facilities, VNA, etc.
- Maintains knowledge and reference materials on key resources available to patients and patients' caregivers across the continuum.
Requirements:
- Associate degree or Bachelor's Degree preferred.
- Healthcare experience, preferably in extended care facilities and community agencies, preferred.
- Preferred experience in hospital discharge planning, long-term care facility, community health, or utilization review preferred.
Skills and Abilities:
- Interpersonal skills to interact effectively with various levels of staff, patients, families, and community organizations.
- Extensive knowledge of regulations, community organization, state and federal systems, medical terminology, and levels of healthcare.
- Ability to manage a variable workload with the ability to constantly change priorities.
Working Conditions:
Works in a busy and, at times, stressful hospital/office environment. Must be able to work well independently and in a multi-disciplinary group.
