Senior Palliative Care Nurse Practitioner

About the RoleThis challenging role will require the successful candidate to work as a Senior Palliative Care Nurse Practitioner at Astera Cancer Care in Edison, NJ. As a key member of the multidisciplinary team, the successful candidate will be responsible for performing office-based and telemedicine palliative care visits, collaborating with healthcare professionals to develop realistic care expectations, and facilitating patient decision-making regarding treatment options.
The ideal candidate will have a strong background in palliative care, with experience working with patients with serious illnesses. They will also possess excellent communication skills, with the ability to effectively communicate with patients, families, and healthcare teams. A graduate degree in Nursing with a specialization in Palliative Care or related field is required. Board certification in Advanced Practice Nursing (APN) is also preferred.
Key Responsibilities- Perform comprehensive assessments and evaluations of patients to identify their needs and develop individualized care plans.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
- Facilitate patient decision-making regarding treatment options, including advance care planning and end-of-life care.
- Prescribe medications and interventions to manage symptoms and improve quality of life.
- Document and bill accurately for services provided.
This exciting opportunity offers a highly competitive salary of $120,000 - $150,000 per year, depending on experience.
