Overview

Signature HealthCARE is excited to offer a specialized training program for new Minimum Data Set (MDS) Nurses, designed to equip Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) with the skills and knowledge necessary to excel in long-term care. This program spans 8 weeks, with training sessions including classroom and handoff at facility level. It provides a unique opportunity to develop expertise in MDS processes, ensuring high-quality care for residents while meeting regulatory requirements.

We are searching for compassionate, detail-oriented, and driven nurses who are eager to make a difference in the lives of our residents. As an MDS Nurse, you will play a critical role in assessing and coordinating care, and this program is designed to prepare you for success in this vital position.

Training Program Structure

Duration: 8 weeks

Schedule: Training will be conducted in classroom and facility level.

Content: The program will cover all aspects of the MDS process, including resident assessments, care planning, documentation, compliance with federal and state regulations, and interdisciplinary team collaboration.

Key Responsibilities

As a MDS Nurse, you will be responsible for:

  • Conducting resident assessments to accurately capture the care needs of residents.
  • Developing individualized care plans based on assessment data to ensure comprehensive care.
  • Gaining a deep understanding of MDS regulations and guidelines to maintain compliance with federal and state requirements.
  • Working closely with other healthcare professionals to ensure holistic resident care.
  • Mastery of precise and timely documentation to support care delivery and regulatory compliance.
  • Demonstrating proficiency in all areas of the Resident Assessment Instrument (RAI), Care Planning, Prospective Payment System (PPS), Medicare, and, when applicable, Medicaid Case Mix.
Program Outcomes

By the end of this program, participants will be able to:

  • Assess patients accurately and evaluate the quality of care for existing residents.
  • Effectively collect, monitor, and document patient care information, utilizing this data to create comprehensive reports that guide care decisions and ensure compliance.
  • Create and implement individualized care plans that include both short-term and long-term goals for resident improvement, tailored to each resident's specific needs and conditions.
  • Collaborate with caretakers, families, and multidisciplinary treatment teams, fostering communication and teamwork to provide the highest quality of care for residents.
Requirements

To be considered for this training program, you must:

  • Hold a current and active LPN or RN license in the state of employment.
  • Have strong organizational skills and attention to detail.
  • Provide excellent service to residents, families, and colleagues.
  • Have effective communication skills.
  • Be willing to engage fully in the 16-week training program and apply new knowledge in practice.
