Healthcare Coordinator for Adults with Disabilities

Job Summary:
We are seeking a skilled LPN to serve as a Nurse Case Manager, coordinating healthcare services for adults with disabilities residing in residential programs.
About the Role:
This is an excellent opportunity for a dedicated and compassionate LPN to oversee general health issues, monitor medication and medical records, consult with MDs, and supervise medication administration.
Duties and Responsibilities:
- Monitor medication and medical records to ensure optimal patient care
- Consult with Medical Doctors to develop personalized treatment plans
- Supervise direct support professional (DSP) staff in medication administration
- Ensure timely medical follow-ups and prescribed medications
- Accompany or transport individuals to appointments when necessary
- Maintain accurate and mandated medical records
Requirements:
- 1-2 years of LPN experience (highly preferred)
- Valid NYS LPN license in good standing
- Associates Degree in Nursing required
- BLS, CPR certified; SCIP-R certification required
What We Offer:
Competitive salary: $65,000 - $85,000 per year, four weeks' vacation, comprehensive benefits, tuition reimbursement, and eligibility for Public Service Loan Forgiveness Program.
