LPN Leader for Resident Care and Wellbeing

At The Grove at Countryside, we are seeking an experienced Licensed Practical Nurse to lead our team in providing exceptional care and wellbeing services to our residents.
We offer a competitive salary of $62,000 - $72,000 per year, along with personalized compensation packages that include top pay, night and weekend shift differentials, loyalty and referral bonuses, and opportunities for spot bonuses and incentive awards.
Job Summary:
The LPN Leader will be responsible for maintaining communications with healthcare providers, ensuring compliance with federal, state, and local laws and regulations, collecting and reporting occurrences, and developing improvement plans. They will also partner with the administrator and other team members to maintain resident and team member health and safety.
Responsibilities:
- Maintains effective communication with healthcare providers regarding recent changes or requests for orders.
- Ensures compliance with federal, state, and local laws and regulations as it pertains to clinical care.
- Collects and reports occurrences, and assists in developing/implementing improvement plans.
- Partners with administrator and other team members to maintain resident and team member health and safety.
- Works closely with pharmacy consultant to provide optimal pharmaceutical services to residents.
- Responsible for infection control practices and safe medication distribution/administration.
- Identifies ongoing needs and services of residents through evaluations and service plan documentation.
- Supervises Certified Nursing Assistants and direct care staff to ensure delivery of high-quality care to residents.
Educational Requirements and Experience:
To be successful in this role, you will need to possess a current state license as a Licensed Practical Nurse, experience in assisted living, home health, or long-term care industries, and at least two years of experience as an LPN. You must also demonstrate competence in assessment skills, injections/medication administration, follow-up, charting, triage, communication, and effective team leading/supervision.
