Job Summary

We are seeking a highly skilled and compassionate Nephrology Care Coordinator to join our team at Metrolina Nephrology Associates. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to individuals with chronic kidney disease (CKD).

Key Responsibilities
  • Provide direct access to CKD patients using Phamily software platform
  • Work closely with nephrologists, advanced practitioners, clinical staff, patient navigators, and ESRD transition coordinators to promote optimal patient experience
  • Coordinate timely healthcare services following hospitalizations, including proactive weekly contact with patients to review current medications and health status
  • Triage and assess patient health status, identify new medications, hospitalizations, and diagnoses
  • Communicate upcoming appointments, access placement, and identify barriers to compliance
  • Update patient charts, assess health needs, and educate patients on late-stage CKD and treatments
Requirements
  • High school diploma or equivalent, completion of medical assistant training program, or graduate of an accredited nursing program
  • Minimum three years' experience in a medical facility as a medical assistant/nurse
  • CMA/RMA certification, current North Carolina nursing license, and CPR certification
  • Epic experience, knowledge of clinical processes, healthcare systems, and community resources
Personal Qualities
  • Professional presentation, ability to work independently, and confidence in influencing patients to participate in their healthcare needs
  • Excellent communication skills, ability to resolve conflict situations, and work collaboratively with physicians, directors, and staff
