Primary Care Nurse Practitioner Opportunity

CVS Health Careers is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Key Responsibilities:
  • Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients
  • Collaborate with the clinical care team to ensure seamless care coordination and communication
  • Develop and implement patient-centered care plans, incorporating evidence-based practices and guidelines
  • Communicate effectively with patients, families, and healthcare professionals to ensure optimal care
  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures
Requirements:
  • Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role
  • Minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
  • Current National Board Certification in Family Practice
