We are seeking a dedicated and compassionate Hospice Care Nurse to join our team in Dallas, TX. As a Hospice Care Nurse with Cantex, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

About This Role

This is a rewarding and challenging position that requires strong clinical skills, excellent communication abilities, and a genuine passion for delivering high-quality patient care. Our ideal candidate will have a minimum of 2 years of experience as a Registered Nurse in a hospice setting and be licensed per state requirements.

Responsibilities
  • Administer nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency.
  • Assume responsibility for coordination of care, ensuring quality and continuity of service provided.
  • Implement/develop/document plans of care to ensure quality and continuity of care and recommend revisions as necessary.
  • Counsel patients and families regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Monitor assigned cases to ensure compliance with requirements of third-party payors.
  • Promote Agency philosophy and administrative policies, providing effective communication to patients, family, team members, and other healthcare professionals.
Requirements
  • Registered Nurse, licensed per state requirements.
  • 2 years of experience as a Registered Nurse in a hospice setting preferred.
  • Good communication skills, assessment skills, and strong clinical documentation skills.
  • Ability to read, write, and speak the English language.
  • Reliable transportation and valid current auto liability insurance.
