Job Description

The County of Contra Costa, CA is seeking a qualified individual to fill the position of Care Manager. In this role, you will be responsible for coordinating patient care and services to ensure that residents receive the necessary support and resources to maintain their health and well-being.

Key Responsibilities:
  • Coordinate patient care with healthcare providers, family members, and caregivers to develop and implement comprehensive care plans.
  • Conduct assessments of patients' physical, emotional, and social needs to identify areas where additional support or resources are required.
  • Develop and implement strategies to address identified needs, including referrals to community-based services, counseling, and education.
  • Collaborate with healthcare providers to ensure continuity of care and make recommendations for follow-up appointments or testing as needed.
Requirements

To be successful in this role, you must have strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations. You should also possess excellent problem-solving and critical thinking skills, with the ability to analyze complex situations and develop creative solutions.

In terms of education, we require a registered nurse degree and/or relevant experience working in a similar field. Additionally, you must have a valid driver's license and reliable transportation, as you may be required to travel to various locations throughout your workday.

Compensation and Benefits

We offer a competitive salary range of $85,000 - $110,000 per year, depending on your level of experience and qualifications. You will also receive a comprehensive benefits package, which includes medical, dental, and vision coverage, as well as a 401(k) plan with employer matching.
