Care Manager for Individuals with Developmental Disabilities

The Opportunity Tree is a non-profit organization dedicated to providing quality individualized services to people with intellectual and developmental disabilities.
Job OverviewThis role requires a Care Manager who will supervise Direct Support Professionals (DSPs) to support Members with developmental disabilities in developing/enhancing basic living and other life skills, promoting independence and individual choice.
- Supervise DSPs' work, schedules, and timekeeping approvals.
- Ensure Members' health, safety, and security by assisting with Activities of Daily Living (eating, bathing, dressing, toileting, ambulation, and continence).
- Maintain monthly activity calendars, ensure they are completed, posted, and up-to-date.
- Responsible for data collection and reporting on a quarterly basis.
- Manage all records/paperwork and submit documentation as per assigned timelines.
- Maintain a safe home environment according to Arizona Licensing Standards/TOT Policy.
- May work alongside DSPs in the performance of their duties.
- Maintain confidentiality of information regarding Members and The Opportunity Tree.
- Advocate on behalf of Members and their families to ensure individuals with intellectual and developmental disabilities have a voice.
To be successful in this role, you must possess:
- Minimum age of 18 years.
- A valid Arizona driver license with a Non-Restricted Motor Vehicle Record and compliance with TOT's insurance carrier requirements.
- Excellent communication skills, including the ability to write, speak, and listen effectively in English.
- Prior experience in the Intellectual and Developmental Disabilities (IDD) field is preferred.
- High school diploma or equivalent education.
- Familiarity with Beginner-level MS Outlook, MS Word, MS Excel, and web browsing.
- A personal enthusiasm for participation in a non-profit organization providing services to individuals with intellectual and developmental disabilities.
- A positive attitude and ability to represent The Opportunity Tree effectively in the community. Ability to work collaboratively as a team member with Members, staff, families, funders, and stakeholders.
- Succesfully meet AZ-DPS requirements for Fingerprint Clearance Card.
- Skills to include knowledge of principles in staff interaction, task productivity, time management, problem identification, and resolution administration.
We offer a competitive salary of $45,000 - $60,000 annually, commensurate with experience, along with benefits such as 401(k), dental insurance, employee assistance program, health insurance, life insurance, paid time off, referral program, and vision insurance.
