Job Summary:

As a Clinician/Case Manager for the Cornerstone Program, you will provide therapeutic and case management services to transition-age youth and their families. This role involves working under professional supervision to deliver community and office-based services, including individual and family therapy, crisis intervention, advocacy, and case management. Your goal will be to assist youth and their families in accessing necessary medical, social, educational, and other services.

Key Responsibilities:

Provide individual and family therapy sessions to youth and their families
Conduct crisis interventions and develop plans to support youth and families
Advocate for youth and families to access necessary services and resources
Develop and implement case management plans to support youth and families
Collaborate with other professionals to ensure comprehensive support for youth and families

Requirements:

Master's degree in social work, clinical psychology, or a related mental health field
One year of experience and training in examining, evaluating, and treating minors and their families
Professional registration or certification (L.M.S.W., L.L.P., RN, etc.)
Valid driver's license and access to a private vehicle for day-to-day job performance
Fluency in Spanish preferred

Competitive salary range: $46,023 to $58,233 (Full-Time/Union)
