Physical Therapist Home Health Specialist

The Primary Physical Therapist in Home Health plays a vital role in managing an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
Key Responsibilities- Completes comprehensive assessments, develops and modifies plans of care in conjunction with patients, caregivers, and physicians.
- Provides treatment visits and discharge planning to facilitate patient engagement and achievement of goals, clinical outcomes, and positive patient experience.
- Communicates handoff information and coordinates with Physical Therapist Assistants, other therapists, and care team members to support patient care throughout home health and the continuum of care.
- Bachelor of Science degree in Physical Therapy.
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- Basic Cardiac Life Support certification within 7 days of employment start date.
