Talent4Health is seeking an experienced Home Health RN to join our team in Rutland, Vermont.

About the Job:

We are looking for a skilled and compassionate nurse who can provide high-quality care to patients in their homes. As a Home Health RN with Talent4Health, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:

  • Assess patient needs and develop personalized care plans to meet their unique requirements.
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records to support quality improvement initiatives.
  • Participate in peer review audits and contribute to the development of best practices within the organization.

Requirements:

  • 2+ years of experience as a Home Health RN
  • Current licensure as a Registered Nurse (RN) in Vermont
  • Strong clinical skills and ability to work independently
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and adapt to changing situations
