Our caregivers carry low patient caseloads, ensuring that each patient receives the attention and support they deserve. Schedule: Part-time, 3-12 hour shifts per week, including rotating weekends

What You'll Do:

  • Deliver direct patient care as defined by the State Nurse Practice Act, prioritizing comfort, dignity, and quality of life
  • Implement current nursing practice based on comprehensive assessments and plans of care, ensuring seamless coordination of services
  • Document patient services accurately and in a timely manner, reflecting the plan of care and promoting effective communication among interdisciplinary teams
  • Provide education and information to patients and families/caregivers, empowering them to take an active role in their care
  • Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary group to ensure comprehensive support
  • Manage pain and symptoms effectively, evaluating patients' responses to treatments and medications

Salary: $65,000 - $75,000 annually, depending on experience

Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings
  • Current licensure as an LPN/LVN in the practicing state
  • CPR certification
  • Demonstrated excellent verbal and written communication, organization, and interpersonal skills
  • Valid driver's license with insured automobile
  • Familiarity with the philosophy of hospice care and its principles
