As a Physical Therapist at Woven Care, you will:

  • Collaborate with the family, physicians, schools, and other educational institutions, and our team to deliver the best clinical outcomes, and ensure the continuum of care for our patients.
  • Complete case management activities consistent with department quality assurance and quality improvement standards.
  • Complete diagnostic evaluations consistent with contemporary, evidence-based practice in physical therapy and department quality assurance standards. Incorporate principles of cultural competence and family-centered care into clinical services.
  • Provide services consistent with contemporary, evidence-based practice in physical therapy and department quality assurance standards.

Requirements:

  • Doctorate from an Accredited PT program
  • BLS for Healthcare Providers/CPR
  • State of Colorado Department of Regulatory Agencies (DORA) Physical Therapist (PT) license; Credentialing must be completed by the Medical Staff Office (MSO) within 3 months of employment

The job requires the ability to sit for 1-2 hours and stand or walk for 4-6 hours daily. It involves frequent bending and occasionally carrying, lifting, pulling, pushing, and reaching, each up to one-third of the time. Precise motor function with both hands is needed, and lifting up to 50 pounds may be required up to one-third of the time.
