Rehab Without Walls Neuro Rehabilitation is a pioneering program that revolutionizes neuro rehabilitation by moving individuals from institutional settings into their natural home and community environment through a continuum of care.

We are seeking an experienced Licensed Clinical Social Worker to join our innovative team in the Greater Sacramento area. As a Care Coordinator Neuro Rehabilitation Specialist, you will play a critical role in providing counseling services that support patients' physical, mental, cognitive, and emotional adjustment through recovery.

Your responsibilities will include:

  • Performing comprehensive evaluations to assess patients' social, emotional, and support needs
  • Sets measurable objectives in conjunction with the rehab team
  • Assisting physicians and other health team members in understanding significant social and emotional factors related to patients' health problems
  • Instructing and counseling patients and families on treating and coping with social and emotional responses connected with illnesses
  • Participating in care coordination activities and acting as a resource to other health team members in identifying and resolving patient needs

To be successful in this role, you will need:

  • A Master's Degree in Clinical Social Work (LCSW)
  • Current Licensed Clinical Social Worker licensure in the state of practice
  • A minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
  • CPR certification
  • The ability to lift 50 pounds and perform other physical requirements listed in the job description
