About the RoleWe are seeking an experienced Physical Therapist to join our team at Creative Health Solutions. The successful candidate will collaborate closely with healthcare professionals to design and administer therapy programs aimed at enhancing patients' physical, mental, and emotional well-being.
Responsibilities- Conduct thorough assessments of patients, reviewing available information to inform treatment decisions.
- Develop and implement individualized treatment plans, setting achievable functional goals based on patient needs.
- Revise treatment plans as necessary throughout the treatment process.
- Document patient progress according to applicable federal and state laws, as well as administrative and clinical requirements.
- Supervise Assistants in accordance with State Practice Act guidelines.
- Orient patients to program services and procedures, informing them of potential risks.
- Communicate effectively with nursing staff, patients, and families to ensure effective care delivery.
- Teach patients and their caregivers exercises and therapeutic procedures to be continued after discharge.
- Assist in the referral process when patients require additional services post-discharge, maintaining current knowledge of community resources.
- Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy with five years of direct practice experience.
- Current license or eligibility for licensure under state statutes in the state of practice.
- Demonstrated ability to apply principles, methods, materials, and equipment used in Physical Therapy to the patient population served.
- Excellent oral and written communication, as well as interpersonal skills.
- Adequate computer skills to complete required job duties and documentation.
- Able to assess patient needs and develop comprehensive plans of care.
- Strong organizational skills to manage multiple projects simultaneously.
- Ability to work independently, requiring creativity and adaptability.
The estimated salary for this role is $85,000-$110,000 per year, depending on experience and qualifications. Compensation packages may include benefits such as health insurance, retirement planning, and paid time off.
