Job Responsibilities
The Home Health Visiting Nurse provides intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities.
The nurse is responsible for communicating patient progress with other disciplines, directing and supervising non-professional home health aide staff, and providing personal care to patients.
Key Duties & Responsibilities:
- Admit patients eligible for home care services within 24-48 hours under physician's orders.
- Assess and evaluate patient needs/problems, identifying mutually agreed-upon goals with patients.
- Report patient status and need for other disciplines to agency clinical supervisor and referring physician.
- Develop patient care plans that specifically address identified patient problems and goals.
- Update care plans on an ongoing basis, revising and resolving patient problems and goals as changes occur or at recertification.
- Provide intermittent skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities outlined in the patient Plan of Care.
- Notify agency of need to alter visit schedule.
- Submit completed skilled nursing visit notes, communication notes, and home health aide supervisory notes per agency policy on designated days.
- Perform all OASIS time point assessments per Medicare criteria and submit recertification paperwork per agency policy and procedure.
- Mainain open lines of communication with all members of the continuum of care team.
- Supervise Home Health Aide and license and document per Medicare criteria and per agency policy and procedure.
- Act as a preceptor in the orientation of new nursing staff as requested.
- Attend staff meetings, team conferences, and educational in-services per agency requirements.
- Participate in Process Improvement (PI) program by assisting with data collection and serving on PI teams as requested.
- Participate in discharge planning process according to Medicare criteria and agency policy and procedure.
- Follow agency policies and procedures.
- Perform additional duties as assigned by the administrator.
- Able to lift 40 pounds from floor to shoulder.
- Repetitive walking, standing, sitting, bending, and use of hands.
- Able to drive a car 2-4 hours per day.
- Responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care.
