Job Responsibilities

The Home Health Visiting Nurse provides intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities.

The nurse is responsible for communicating patient progress with other disciplines, directing and supervising non-professional home health aide staff, and providing personal care to patients.

Key Duties & Responsibilities:

  1. Admit patients eligible for home care services within 24-48 hours under physician's orders.
  2. Assess and evaluate patient needs/problems, identifying mutually agreed-upon goals with patients.
  3. Report patient status and need for other disciplines to agency clinical supervisor and referring physician.
  4. Develop patient care plans that specifically address identified patient problems and goals.
  5. Update care plans on an ongoing basis, revising and resolving patient problems and goals as changes occur or at recertification.
  6. Provide intermittent skilled nursing services, including assessment, evaluation, procedures, teaching, and training activities outlined in the patient Plan of Care.
  7. Notify agency of need to alter visit schedule.
  8. Submit completed skilled nursing visit notes, communication notes, and home health aide supervisory notes per agency policy on designated days.
  9. Perform all OASIS time point assessments per Medicare criteria and submit recertification paperwork per agency policy and procedure.
  10. Mainain open lines of communication with all members of the continuum of care team.
  11. Supervise Home Health Aide and license and document per Medicare criteria and per agency policy and procedure.
  12. Act as a preceptor in the orientation of new nursing staff as requested.
  13. Attend staff meetings, team conferences, and educational in-services per agency requirements.
  14. Participate in Process Improvement (PI) program by assisting with data collection and serving on PI teams as requested.
  15. Participate in discharge planning process according to Medicare criteria and agency policy and procedure.
  16. Follow agency policies and procedures.
  17. Perform additional duties as assigned by the administrator.
  18. Able to lift 40 pounds from floor to shoulder.
  19. Repetitive walking, standing, sitting, bending, and use of hands.
  20. Able to drive a car 2-4 hours per day.
  21. Responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care.
