Job Title: Clinical Care Coordinator

About the Role:

Northern Light Health is seeking a highly motivated and compassionate Registered Nurse I to join our team in Sebasticook, ME. As a Clinical Care Coordinator, you will play a vital role in delivering high-quality patient care and contributing to a healthy work environment.

About the Opportunity:

This full-time position offers a 36-hour week with alternate shift schedules (day, evening, nights, weekends). The ideal candidate will have excellent communication skills, be able to work effectively in a team environment, and prioritize tasks to meet deadlines.

Responsibilities:

  • Provide safe and effective patient care by using best practices during hand-off communication and when reporting patient care information.
  • Demonstrate knowledge and skills in location, operation, and care of equipment.
  • Perform and document systematic assessments according to established criteria.
  • Recognize and respond to abnormal diagnostic data and report to appropriate providers.
  • Develop and maintain professional relationships with patients, families, and staff.
  • Treat patients with courtesy and respect.
  • Utilize theoretical knowledge base to operationalize care.
  • Accurately document patient care information reflecting adherence to hospital, regulatory, and legal requirements.
  • Complete population-specific mandatory education competencies as defined by the unit scope of service.
  • Provide culturally competent care to patients and families.
  • Practice is consistent with Northern Light Health policy and procedures.
  • Demonstrate awareness of evidence-based practice.
  • Communicate effectively with patients, family members, and interdisciplinary teams.
  • Implement plans of care and identify resources to meet patient needs.
  • Contribute to a healthy work environment by exhibiting professional behaviors.

Credentials and Qualifications:

We are looking for a Registered Nurse with a High School Diploma/General Educational Development (GED) and a strong passion for delivering exceptional patient care. BLS certification is required.

Working Conditions:

The Registered Nurse I will work in a fast-paced healthcare environment, lifting, moving, and loading up to 50 pounds. The successful candidate will be able to adapt to changing priorities and manage multiple tasks simultaneously.

Estimated Salary: $62,500 - $72,500 per year, depending on experience
