Job Summary

We are seeking a skilled Healthcare Advocate to join our team at Four Rivers Community Health Center. As a key member of our staff, you will play a vital role in helping patients navigate the healthcare system and access the resources they need to live healthy, fulfilling lives.

Key Responsibilities:

  • Assist patients in accessing community services and adopting healthy behaviors
  • Help eligible clients apply for publicly sponsored health care insurance plans and provide close follow-up throughout the approval process
  • Conduct outreach in the community to promote, maintain, and improve individual and community health
  • Provide information on available resources, social support, and informal counseling to individuals and community members
  • Collect data to help identify community health needs and conduct continuous process improvement as part of the quality assurance program

Requirements:

  • High School diploma or equivalent required
  • Experience with social services or public health system is preferred
