Job Summary

The Hospice Social Worker is a key member of our interdisciplinary team, responsible for delivering social work services to hospice patients and their families. This role requires a strong understanding of hospice care and the ability to work collaboratively with patients, families, and healthcare providers.

Responsibilities
  • Conduct psychosocial evaluations and develop plans of care for patients and their families
  • Provide counseling and education to patients and families, addressing their physical, emotional, and spiritual needs
  • Coordinate referrals and bereavement services as needed
  • Participate in interdisciplinary team meetings and contribute to the development and evaluation of patient plans of care
  • Assist patients and families in determining goals of care, including completion of Advance Directives and POLST forms as needed
Requirements
  • Master's degree in social work from an accredited college
  • Current Oregon LCSW or LMSW license
  • Minimum one year hospice experience, including a background in medical social work
  • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
  • CPR certification at hire or within 6 months in position
