Job Summary

We are seeking a highly skilled and compassionate Nursing Home Team Manager to join our team at VITAS Healthcare. As a key member of our interdisciplinary team, you will be responsible for directing patient care services, ensuring seamless coordination of care, and promoting a culture of excellence and compassion.

Key Responsibilities
  • Direct patient care services, including volunteer and bereavement support, to ensure high-quality care and patient satisfaction.
  • Collaborate with team physicians and attending physicians to develop and implement palliative care plans, ensuring timely and effective pain and symptom management.
  • Review patient admissions, assessments, and discharges to ensure eligibility and appropriateness criteria are met, and make recertification decisions in a timely manner.
  • Monitor team services, including staffing, medications, HME, supplies, and level of care changes, to ensure they are responsive to patient and family needs.
  • Manage team schedules and coordinate services 24/7 to ensure seamless care delivery.
  • Facilitate team meetings, ensuring interdisciplinary input into the plan of care and promoting a culture of collaboration and excellence.
  • Participate in on-call rotation and provide support to team members as needed.
Staff Supervision and Management
  • Interview, select, train, supervise, evaluate, and dismiss team staff in conjunction with the Patient Care Administrator.
  • Ensure staff competence and performance levels through field visits, documentation review, and provision of team-based in-services on VITAS values, hospice principles, and practices.
  • Act as a resource and mentor for staff on clinical issues, documentation, inter- and intra-team problem-solving, and customer service behavior.
  • Oversee staff and volunteer schedules, scheduling, and territory assignments to ensure equitable workload distribution and meet productivity expectations.
Quality Improvement/Regulatory Compliance
  • Monitor clinical records and team activities to ensure accuracy, completeness, and compliance with VITAS policy and regulatory requirements.
  • Develop and implement performance improvement activities to respond to service issues and challenges.
  • Ensure team completion of proactive and other patient/family satisfaction surveys and implement Quality Improvement measures and standards.
  • Assist with surveys and requests from regulatory bodies and intermediaries as directed by local program.
Customer Service/Sales/Marketing
  • Address patient/family and physician/MCO concerns and resolve issues promptly and satisfactorily.
  • Analyze customer service issues to identify causes and work with team members to improve performance.
  • Personally engage with patients/families and attending physicians to resolve issues and prevent revocation.
  • Regularly visit LTC and Contract Bed facilities to ensure care plan integration, customer satisfaction, and discuss appropriateness criteria.
Cost Containment
  • Verify and approve admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitor resource utilization to ensure cost-effective delivery of services.
  • Control and account for productivity, labor, and related patient care costs within budget.
  • Approve all bills/invoices related to patient care services.
Professional Development
  • Attend inservices, educational seminars, and workshops to enhance professional growth and development.
  • Develop and achieve professional growth goals and objectives.
  • Participate as a mentor for newly hired Team Managers.

Requirements:

  • Reliable transportation with appropriate license and insurance coverage.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care principles, oncologic nursing, and palliative care.
  • Basic understanding of interpersonal dynamics within interdisciplinary healthcare teams and the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and management reports.
  • Two years successful supervisory experience or equivalent in a healthcare organization.

Education: Bachelor's degree preferred. Current and valid R.N. License to practice in the state where the VITAS program is located.
