Job Summary

The Home Health Licensed Practical Nurse provides nursing care to patients and families in their homes. The scope of nursing includes direct patient care, patient advocacy, and coordination of care with the multidisciplinary team.

Essential Functions

  • Assess patients' physical and psychosocial needs to determine health status within the scope of licensure.
  • Collect relevant data and document information according to Medicare record policy and procedure.
  • Complete all nursing documentation per procedures and nursing standard of practice, including assessments, interventions, responses to interventions, communications, and verbal orders.
  • Derive a holistic nursing diagnosis from comprehensive patient assessment and prioritize same according to patient need.
  • Follow the nursing care plan established by the RN Case Manager, based on nursing diagnoses that includes measurable, realistic, and time-directed goals and nursing interventions specific to the problem and individual patient.
  • Provide therapeutic care, education, and referral, seeking maximum patient and family participation and independence.
  • Assess patient needs relative to HHAs and communicate with team RNs and HHA Manager.
  • Develop, maintain, and apply a knowledge of third-party payer guidelines governing eligibility for reimbursable home health services.

Qualifications

  • Active LPN licensure in Massachusetts and New Hampshire (or willingness to obtain).
  • Minimum of one year of acute nursing experience.
  • Home care nursing experience preferred.
  • Must have a driver's license, auto insurance, and current CPR certification.
