Key Responsibilities:
Collaborate with the care management team to deliver coordinated patient care across the continuum.
Provide psychosocial clinical assessment, intervention, evaluation, consultation, and education to patients and families as part of comprehensive care management services.
Identify high-risk patients from a psychosocial and financial perspective and provide information, support, counseling, care management, and referrals to appropriate resources.
Serve as a liaison for complex transitions and discharges between the hospital and community agencies for the exchange of clinical and referral information.
Requirements:
Master of Social Work degree.
Strong communication and interpersonal skills.
