Nurse Practitioner

About the Role:

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Ascension Living, providing high-quality care to patients in our long-term care setting.

Salary and Benefits:

The salary range for this position is $43.00 - $47.00 per hour, with opportunities for advancement and professional growth. Responsibilities:

• Provide direct nursing care to patients in the long-term care setting, implementing and monitoring patient care plans as needed.">
• Assess and coordinate patient's discharge planning needs with members of the healthcare team, ensuring a seamless transition to home or further care.">
• Note and carry out physician and nursing orders, maintaining accurate records and communicating effectively with the healthcare team.">
• Implement safety protocols and identify potential hazards, taking corrective action as necessary to ensure resident safety.">
• Serve as a primary coordinator of all disciplines for well-coordinated patient care, collaborating with healthcare professionals to achieve optimal outcomes.">
• Monitor, record, and communicate patient condition as appropriate, updating care plans accordingly.">
• Note and implement physician and nursing orders, coordinating care with other healthcare professionals.">

Requirements:

To be successful in this role, you will need to possess:

• A BLS Provider certification obtained prior to hire date or job transfer date.">
• A Registered Nurse certification obtained prior to hire date or job transfer date.">
• A diploma from an accredited school/college of nursing required.">
