Senior Medical Care Specialist

As a Nurse Practitioner at Abode Care Partners, you will play a pivotal role in delivering high-quality medical care to seniors and patients with complex needs. This role involves conducting thorough assessments of patient medical needs, developing and implementing evidence-based treatment plans, and collaborating with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:
- Evaluate patient medical needs and develop individualized treatment plans
- Treat chronic and urgent illnesses, minor injuries, accidents, and other health issues
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care
Requirements:
- Active state NP license
- Current APRN certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- At least two years of experience as a Nurse Practitioner
