At Optum, we're committed to creating a holistic approach that addresses the physical, mental, and social needs of our patients. Our Senior Community Care team is seeking an experienced Nurse Practitioner or Physician Assistant to join our ranks.

Job Summary

We offer a market-competitive compensation model with salary and incentive opportunities, ranging from $140,000 to $180,000 per annum, depending on experience. This position involves providing comprehensive assessments, managing acute medical situations, and ensuring accurate documentation. The ideal candidate will have a strong understanding of geriatrics, chronic illness, and advanced illness discussions.

Key Responsibilities:
  • Conduct thorough patient evaluations to identify health risks and develop personalized care plans.
  • Manage complex cases, including testing, diagnostics, and interventions, in collaboration with the interdisciplinary care team.
  • Maintain accurate and complete electronic medical records, adhering to ICD 10 coding standards.
  • Communicate effectively with patients, families, and caregivers to ensure optimal care outcomes.
  • Participate in advanced illness and end-of-life discussions, offering guidance and support to patients and their loved ones.
Requirements:
  • A valid and unrestricted Nurse Practitioner or Physician Assistant license in the state of residence.
  • A graduate degree in nursing or a related field from an accredited institution.
  • Board certification through the American Academy of Nurse Practitioners or the National Commission on Certification of Physician Assistants (NCCPA).
  • Active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • The ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
