Job Title: Clinical Support Coordinator

At Commonwealth Care Alliance, we are seeking a highly organized and detail-oriented Clinical Support Coordinator to join our team. As a key member of our care team, you will provide administrative support to ensure the smooth delivery of high-quality patient care.

Key Responsibilities:
  • Coordinate outgoing referrals to specialists and VNA agencies, ensuring timely and accurate communication.
  • Schedule patient appointments, including new patient visits, annual physicals, and hospital discharge follow-ups.
  • Provide front desk support, including checking patients in and out, completing insurance eligibility checks, and contacting transportation as needed.
  • Draft correspondence, such as Jury Duty letters and Letters of Medical Necessity, as requested by the care team.
  • Monitor incoming faxes and save documents appropriately in the medical record, routing them for review and completion.
  • Complete reminder calls for upcoming visits and fax documents and/or prescription orders to pharmacies as requested.
  • Conduct test calls via telehealth technologies to ensure patients can connect successfully for appointments.
Requirements:
  • High School Diploma or GED
  • 1-2 years of related experience in a healthcare or medical office setting
  • Ability to prioritize workload and manage multiple projects simultaneously
  • Microsoft Office skills, including Outlook, Excel, Word, and PowerPoint
  • Excellent written and verbal communication skills
  • Knowledge of medical terminology and Electronic Medical Records (EMRs)
