Position Overview

Role Summary:
The Qualified Professional for Community Support Team is tasked with the strategic planning, coordination, and management of therapeutic and rehabilitative services for individuals enrolled in Community Support Team Services. This role involves the initial and continuous development of the client's Person-Centered Plan (PCP), as well as the implementation and coordination of services aligned with the PCP. The Qualified Professional will conduct ongoing assessments of client needs, make necessary revisions to the PCP, evaluate the effectiveness of interventions, and measure client outcomes. Additionally, the Qualified Professional will provide case management, supportive counseling, and skill-building interventions. Availability for evening and weekend hours is required for all members of the Community Support Team.

Qualifications:
  • Master's degree in a human services field with a minimum of one year of full-time, post-graduate experience in mental health, developmental disabilities, or substance abuse with the target population; or
  • Bachelor's degree in a human services field with at least two years of full-time experience in mental health, developmental disabilities, or substance abuse with the target population.
