About the Role

We are seeking a highly skilled Psychosocial Support Specialist to join our team at Fresenius Medical Care.

Job Description

This is an entry-level MSW role that involves providing psychosocial services to patients treated by the facility, including in-center and home dialysis patients. The successful candidate will work with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for our patients.

Patient Assessment / Care Planning / Counseling
  • The Psychosocial Support Specialist will participate in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
  • They will utilize FKC patient education programs, established social work theory and methods, social work-focused interventions, and quality-of-life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
Quality

The Psychosocial Support Specialist will provide psychosocial support and/or social work-focused interventions to address non-adherence, quality outcome, and quality-of-life concerns for all patients based on acuity level.

Education and Training

The ideal candidate will have a Master's degree in Social Work (MSW) and excellent communication and interpersonal skills.

Salary and Benefits

The estimated salary for this role is around $63,000 per year, depending on experience and location. Benefits include health insurance, retirement plans, and paid time off.
