The Nurse Care Specialist at Ambercare is responsible for delivering high-quality patient care in the comfort of the patient's home. This role involves developing, implementing, and evaluating individualized patient care plans that meet the unique needs of each patient.

The ideal candidate will have a strong background in nursing and excellent communication skills, with the ability to work effectively with patients, families, and healthcare teams. As a Nurse Care Specialist, you will be responsible for providing direct patient care, including observation, assessment, and health teaching, as well as coordinating with other healthcare professionals to ensure seamless care.

Key Responsibilities:

  • Develop and implement individualized patient care plans that meet the unique needs of each patient.
  • Provide direct patient care, including observation, assessment, and health teaching.
  • Coordinate with other healthcare professionals to ensure seamless care.
  • Communicate effectively with patients, families, and healthcare teams.
  • Document patient care and progress in accordance with agency policies and procedures.

Requirements:

  • Current nursing licensure in the state of employment.
  • Minimum of six months recent work experience as a home care nurse or equivalent.
  • Graduate of an approved School of Nursing.
  • Satisfactory references and completion of Agency RN Qualifying Exam.
  • Current CPR certification.

Physical Requirements:

  • Regularly required to sit, stand, and walk on variable surfaces.
  • Regularly required to use hands to finger, handle, or feel, and talk or hear.
  • Occasionally required to reach beyond mid-line of body overhead/chest level.
  • Occasionally use hand controls including grip using one or both hands.
  • Occasionally climb ramps or stairs or use foot controls.
  • Specific vision abilities required by this job include close, distance, and peripheral vision, depth perception, and ability to adjust focus.
  • Ability to lift, push, pull, and carry a minimum of 50 lbs.
  • Ability to stoop and bend up to 2.5 hours per 8 hour shift.

Education/Qualifications:

  • Minimum of six months recent work experience as a home care nurse or equivalent.
  • Current nursing licensure in the state of employment.
  • Graduate of an approved School of Nursing.
  • Satisfactory references and completion of Agency RN Qualifying Exam.
  • Current CPR certification.
