The Maternity Care Management Care Navigator plays a vital role in supporting the care management team by facilitating research and navigation of members to appropriate medical, behavioral health, or community resources.
Key Responsibilities:- Conduct referral management of cases, care management pre-screening, patient scheduling, patient consultation, and transition management to facilitate effective and efficient operations of the care management and utilization management team.
- Provide operational support and transition duties for care management services for identified highest risk (complex) SelectHealth members by working collaboratively with the care management team, patients, family caregivers, significant others, healthcare providers, payers, community-based providers, and other involved parties to provide effective, efficient, and patient-centered care management services.
- Work collaboratively with the care management team to ensure seamless transitions and effective care coordination.
- Develop and maintain relationships with healthcare providers, payers, and community-based providers to ensure access to necessary resources and services.
- Provide education and support to patients and families to promote healthy behaviors and self-management of chronic conditions.
- Experience working successfully in a remote environment or using Advanced Microsoft Suite, including Teams (chat, whiteboard, task tracking) & Outlook.
- Capable of troubleshooting internet connection, basic computer hardware set-up, ability to customize computer settings, and use multiple monitors.
- Demonstrated experience and proficiency working in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
- Demonstrated excellent interpersonal and communication skills.
- Knowledge of medical issues.
- Ability to represent the department with other agencies.
- Demonstrated problem-solving skills.
- Ability to adapt quickly as needs arise.
- Knowledge of available health resources.
- Maternity experience.
- Care Navigator experience.
- Spanish Speaking.
This is a hybrid position with in-office requirements. The hourly range for this position is $21.98 - $33.48. Intermountain Health is an equal opportunity employer.
