Job Summary

Constructive Partnerships Unlimited is seeking a dedicated Habilitation Assistant to join our team at our Queens Long Island City Day Program. As a Habilitation Assistant, you will play a vital role in ensuring the health, safety, and wellbeing of individuals with intellectual and developmental disabilities who attend our program.

Key Responsibilities

Implement and monitor program plans for individuals with intellectual and developmental disabilities.
Monitor activities and ensure they are consistent with valued outcomes of independence, individualization, and productivity.
Ensure daily documentation is maintained and use various software for data collection.
Work directly with individuals who attend the program, assisting with feeding, personal hygiene, and lifting/transferring as needed.

Requirements

High School Diploma and/or GED from an accredited school.
1 year experience working with people with intellectual and/or developmental disabilities.
Excellent communication and interpersonal skills.
Ability to perform data entry on a personal computer (PC). MS Word a plus.
Criminal background check.
Valid NYS Driver's License preferred.
Will be required to successfully pass a Medication Administration Program.
