We are seeking a skilled Hospice Nurse Practitioner to join our team at SouthernCare, part of the Gentiva family of hospice, home health, palliative, and community care providers.
Key Responsibilities- Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Conduct physical assessments for F2F encounters within regulatory time frames established by CMS.
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician.
- Make recommendations to the IDG based on findings of the F2F assessment.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
