We are seeking an experienced physical therapist to join our team at Fairview Health Services in Edina. In collaboration with patients and families, the physical therapist is responsible for delivering high-quality patient care within their scope of practice.
- This includes conducting thorough examinations, evaluating patient needs, developing personalized treatment plans, and implementing interventions to achieve optimal outcomes.
This casual position offers a flexible schedule with opportunities to pick up additional shifts as needed. Ideal candidates will have the capacity to increase their FTE dependent on clinic demands.
M Health Fairview Rehabilitation provides a broad range of services across 10 acute-care hospitals, 3 post-acute settings, and 70 outpatient adult and pediatric therapy clinics. Our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.
We have a collaborative culture centered on learning, emphasizing evidence-based, patient-centered care. Our continuing education program offers over 40 courses per year at no cost to employees, supporting further external CE courses annually.
ResponsibilitiesThe Physical Therapist partners with patients and families while performing evaluation and re-evaluation, capturing pertinent data, and resolving treatment diagnoses.
Key responsibilities include:
- Developing individualized plans of care through coordination
- Analyzing data to identify needs and potential risks, establishing prognosis for recovery, and setting individualized goals
- Implementing and modifying care plans based on reassessment and patient response
- Coordinating care and communicating effectively with interdisciplinary teams for all aspects of care and establishing appropriate discharge plans
- Providing skilled interventions and adapting treatment to reflect changes in patient status and response
- Assessing patient and/or caregiver learning needs and providing appropriate education
To be considered for this role, you must have:
- Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN
- Current Basic Life Support (BLS) certification
Prior experience as a physical therapist is preferred, particularly those with a Doctorate degree in Physical Therapy.
Compensation and BenefitsWe estimate the salary for this position to be around $45,000-$60,000 per year, depending on qualifications and experience.
