About This Role: As a Population Health Care Coordinator at Community Health of South Florida, Inc., you will play a crucial role in improving the health outcomes of patients within disease-specific populations.

Job Summary: We are seeking an experienced Licensed Practical Nurse II to coordinate care and work towards achieving patient outcome goals as established in the Section 330 Health Care Plan. The ideal candidate will have strong communication and interpersonal skills, with the ability to work independently and as part of a team.

Responsibilities:

  1. Coordinate home visits for non-compliant patients
  2. Assist families in accessing healthcare services and assigning them to a medical home
  3. Communicate effectively and express ideas clearly, actively listening and following appropriate channels of communication
  4. Maintain flexibility to access families during non-traditional hours
  5. Collect and report results of outcomes and referrals
  6. Maintain accurate records of activities performed
  7. Follow-up and track missed appointments
  8. Track coordination of referrals for outcome results
  9. Provide documentation of any patient contact when abnormal results are followed-up
  10. Attend scheduled in-services and training courses
  11. Work to achieve patient outcome goals as established in the Section 330 Health Care Plan

Requirements:

  • Graduate from an accredited college, university, or vocational/technical school with LPN license
  • Two (2) years' experience in a healthcare or related setting
  • Experience in program development, project planning, and implementation
  • Experience working with culturally diverse clients
  • Licensed to practice as an LPN in the State of Florida
  • Maintain current CPR certification from the American Heart Association
  • Valid Florida Driver's License

Preferred Qualifications:

  • Excellent computer skills, including proficiency in practice management and EHR software programs
  • Able to navigate and master new software systems quickly
  • Ability to prioritize tasks, multi-task, and achieve goals on time
  • Strong problem-solving and solution-oriented skills

Estimated Salary: $60,000 - $80,000 per year, depending on experience.

Location: South Florida, FL.
