We are seeking a highly skilled Hospice Music Therapist to join our team at Masonicare Home Health & Hospice in the New Haven, CT area.

About the Position

This is a per-visit position, compensated at a rate of $35/visit. As a Hospice Music Therapist, you will provide direct services to patients and their families in homes and inpatient and outpatient units.

Our ideal candidate will have a minimum of a Bachelor's degree, with a Master's preferred, and be Board Certified as a Music Therapist. Two years of experience in end-of-life issues is also desirable.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans, documenting progress and adjusting interventions as necessary.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Design and implement music sessions tailored to meet the unique needs of individuals and groups, promoting emotional well-being and quality of life.
  • Evaluate patient progress regularly, taking prompt action when changes occur or goals are met, and terminating care when appropriate.
  • Promote professional growth and competency through ongoing education and adherence to universal precautions and infection control policies.
Requirements
  • Minimum of a Bachelor's degree; Master's preferred.
  • Graduate of an approved music therapy program; Board Certified as a Music Therapist.
  • A minimum of two years experience and work in the area of end of life issues preferred.
  • Maintains current Drivers' License and auto insurance.
  • Possess reliable transportation.

  • Demonstrates commitment to professional growth and competency in fulfilling position requirements.
  • Demonstrates an understanding of the differences between adult and children's reaction to grief and life-threatening situations.
  • Demonstrates ability to adhere to universal precautions and infection control policies at all times.
  • Demonstrates ability to promote the professional image by adhering to the established dress code of the hospital and wearing name badge as evidenced by supervisor's observation.
