Job Summary:

We are seeking a skilled and compassionate Pediatric Care Coordinator to join our team at Northwell Health. As a key member of our home health services, you will play a vital role in providing high-quality care to patients and their families.

About the Role:

The Pediatric Care Coordinator is responsible for managing all aspects of patient care related to services provided in the home. This includes conducting home visits, completing required documentation, communicating with primary care physicians, updating medication profiles, and coordinating care with other disciplines.

Responsibilities:

• Conduct home care assessments to determine patient eligibility for services
• Complete comprehensive assessments utilizing HCHB
• Develop and implement plans of care based on physical, psycho-social, and environmental assessments
• Provide skilled nursing care as described in the 485-Plan of Treatment
• Orient and supervise home health aide personnel in accordance with regulatory requirements
• Evaluate the effectiveness of interventions in accordance with the plan of care

Requirements:

• Graduate of an approved school of professional nursing
• Currently licensed RN in NYS
• Home care experience preferred
• Excellent verbal and written communication skills

Working Conditions:

This position requires the ability to operate medical equipment, perform physical and psychosocial assessments, and travel to patient homes. Occasional heavy physical exertion may be required, including patient transfers and lifting.

Compensation:

Estimated salary: $85,000 - $110,000 per year, depending on experience. Additional benefits include competitive pay, comprehensive insurance coverage, and opportunities for professional growth and development.
