Job Summary

We seek an experienced Registered Nurse to join our team as a Medical Care Coordinator. The successful candidate will be responsible for planning and delivering high-quality patient care in multiple specialties.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care plans for assigned patients.
  • Organize patient care priorities effectively, considering varying acuity levels.
  • Maintain continuity through clear communication and documentation.
  • Develop comprehensive educational plans for patients and families, utilizing community resources when needed.
Requirements
  • License to practice as a Registered Nurse in Massachusetts required.
  • At least 2 years of experience as a RN required.
  • Epic experience preferred.
  • Strong observational skills and ability to set priorities.
  • Excellent interpersonal and communication skills.
Salary and Benefits

We offer a competitive salary range of $93,000 - $110,000 per year, depending on experience. Additional benefits include opportunities for professional growth and development, a collaborative work environment, and a comprehensive compensation package.
