Job Summary

This position involves working with patients who require behavioral health services, developing individualized care plans, and collaborating with interdisciplinary teams to ensure seamless delivery of care. You will be responsible for assessing patients' needs, identifying treatment goals, and implementing effective interventions to promote optimal outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' behavioral health needs
  • Develop and implement personalized care plans tailored to meet patients' unique requirements
  • Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to coordinate care and ensure continuity of services
  • Provide education and support to patients and their families on various aspects of behavioral health, including symptom management, medication adherence, and coping strategies
  • Document all interactions and progress notes accurately and timely
Requirements
  • Bachelor's Degree in a relevant field (e.g., psychology, social work, nursing)
  • Minimum 1-2 years of experience in behavioral health case management or a related field
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks
  • Strong knowledge of evidence-based practices and treatment modalities in behavioral health

Salary and Benefits

The estimated salary for this position is $65,000 - $85,000 per year, depending on qualifications and experience. Additionally, you will receive a comprehensive benefits package, including medical, dental, and vision insurance, paid time off, and retirement savings plan.
