Clinical Health Advocate

Eagle Physicians is seeking an experienced and detail-oriented Clinical Health Advocate to join our team. As a key member of our primary care team, you will serve as a clinical health advocate and work with a targeted group of patients to support the primary care team in ensuring physicians operate at the top of their licenses while ensuring patients receive preventive care.
Responsibilities- Pre-Visit Planning: Identify patients who have an upcoming appointment, identify gaps in care, and initiate standing orders as appropriate.
- Annual Medicare Wellness Visits (AMW): Identify and possibly schedule patients who are eligible for a Medicare Annual Wellness Visit, educate these patients on the importance of these visits, conduct AWVs, and refer patients to physician as necessary.
- Reviews and interprets provider level data on quality improvement activities, including evaluation of the effectiveness of interventions and adjusting plans as needed.
- Collect and review pharmacy reports to determine whether patient is adhering to medication regimen, work with patient to identify and resolve any barriers to their medication adherence.
- Operate medical equipment, maintain neat and clear work areas, and restock supplies as needed.
- Minimum 3 years prior experience in a medical office preferred.
- Excellent verbal communication skills and problem-solving skills required.
- Strong desire to help and ability to work with diverse populations essential.
- Friendly, approachable, and self-confident demeanor necessary.

Benefits- Competitive hourly rate ($25-$35 per hour) based on experience.

Generally, Monday - Friday 8 AM-5 PM schedule with flexibility to change as indicated by manager.
